This may be the wrong place to post this!

Congratulations to Medicare and Medicaide for refusing to pay Hospital for their mistakes like taking out wrong organ or where patient gets infection in Hospital and will not allow the Hospital to bill the Patient, it about time the Hospitals are being held accountable

More important is that the institutions and persons involved learn to examine the PROCESSES within their Systems that allows or promotes errors or less than optimal treatments. The CMMS is still in the old mindset of kill the messenger rather than look at the message within and learn from it to improve.

The "never events" are:

Wrong-site surgery
Wrong-patient surgery
Wrong-procedure surgery
Unintended foreign object left in the patient
Surgical death in an otherwise healthy patient
Contaminated drug/device
Misapplied therapeutic device
Air embolism
Infant discharged to unauthorized recipient
Elopement with serious consequences/death
Suicide attempt with serious consequences/death
Wrong medication error (time, type, dosage, etc.) with serious results
Blood transfusion typing mismatch
Wrong sperm/egg in test tube baby procedure
Maternal death in a low-risk pregnancy
Death/disability from some cases of low blood sugar
Death/disability from hyperbilirubinemia of a neonate
New stage 3 or 4 pressure ulcers
Death/disability because of spinal manipulation
Serious or fatal electric shock
Contaminated or incorrect medical gas
Serious or fatal burn
Fatal fall
Death or serious disability from use of restraints
Somebody impersonating a care provider
Abduction (patient of any age)
Sexual assault of a patient on hospital grounds
Assault on patient or staff on hospital grounds

There are 28 defined "never events" - Medicare has announced they don't want to pay for them or allow the patient to be billed for them.

The Leapfrog Group (an association of organizations that buy health care for about 34 million people) is working towards the same goal. They're going to be nudging their covered lives towards hospitals that have a strong position on never events, including a "we don't bill for them" provision.
