Hello GeorgeNews. Total brake failure (e.g. front & rear) is a rarity these days. The introduction of a dual master cylinder by American Motors in the 1960s isolated each system, but not entirely as the master cylinder can fail. Another source of failure can be the loss of vacuum to the power brake booster, but you can usually stop a car with manual intervention (e.g. a heavy foot on the brake pedal). Routine inspection is the best cure for the loss of hydraulic brake fluid, DOT 3 in vehicles. I you have total loss of hydraulics, the manual parking/emergency brake is your only option, other than outdriving the situation.
In answer to your Topic question, yes I have lost brakes, but I worked in a garage where people always didn't tell you of their automotive problems and that was back in 1968-1969.

I have not had a BRAKE failure for many years. All of my brake failures were on pre 1968 single circuit system cars. I have had 4 failues of varying severity. I will admit there is a high pucker failure to these events. Fortunately all were manual transmission cars with functional parking brakes and the parking brake combined with the engine/transmission braking I was able to make emergency stops. Definitely not for the faint of heart!!!!!

No, but I do all my own work safely and thoroughly. Losing brakes is no accident, it's neglect. A leak won't cause an immediate loss of stopping power, unless the problem is ignored too long.

I have only experienced this with pre-1968 cars that were single circuit but I lost them at least 4 times that I can remember and it is scary. I never had an accident though and that was kind of a miracle and some quick action.
